If you cannot print a copy of the schedule you can request a hard copy of the schedule to be forwarded in the post by either emailing your request to eprovops@hba.com.au or faxing your request to (03) 9937 4419.

Please include practice ID number or registered Provider number in the Subject field of fax or email requests.
